Urinary Diversion

Reconstructive procedures aimed at restoring urinary drainage play a vital role in modern urological practice, particularly following radical cystectomy (removal of the urinary bladder) for malignant tumors, as well as in patients with severe impairment of urinary tract function.
The goal of these high-technology reconstructive interventions is to create a reliable, physiologically appropriate, and максимально comfortable urinary diversion that enables patients to maintain their usual quality of life.

The optimal surgical approach is selected on an individual basis, taking into account the clinical situation, anatomical characteristics, and the patient’s personal preferences.

At the Department of Urology of the Medical Center - University of Freiburg, such procedures are performed by a team of experienced specialists with a high level of expertise in urological oncology and reconstructive surgery.

Treatment Options

• Ileal conduit (creation of an artificial urinary diversion)

  • Formation of a new urinary drainage pathway using a segment of the ileum
  • Anastomosis of the ureters to the isolated bowel segment, which is brought to the body surface
  • Urine drainage through an external stoma using a specialized urostomy appliance

• Neobladder reconstruction

  • Creation of a new urinary bladder from a segment of the small intestine
  • Connection of the newly formed reservoir to the urethra, allowing natural voiding via the urethra
  • Requirement for adaptation and targeted training to restore and control the function of the new bladder

• Continent internal urinary reservoir with catheterization via stoma

  • Creation of an internal reservoir from a segment of the intestine
  • Urinary drainage through a dedicated stoma using intermittent catheterization
  • No need for an external urine collection bag

Advantages of Reconstructive Urinary Diversion

  • Restoration of natural or functional ability to void
  • Improved quality of life through individualized selection of the urinary diversion technique
  • Faster recovery and a low risk of complications when modern minimally invasive approaches are used

Preoperative Preparation and Postoperative Follow-Up

  • Comprehensive preoperative counseling and shared decision-making regarding the method of urinary diversion
  • Regular postoperative follow-up examinations to assess healing and ensure early detection of potential complications
  • Individually tailored rehabilitation programs aimed at maintaining urinary function and preventing recurrence
